Job Summary:
Episcopal Church Home our 131 bed Skilled Nursing Facility has a rare opportunity for a Social Worker to join our Social Service team. This position will manage the Social Services for our long-term residents.
Job Duties:
· Coordinates social services for the resident
· Creates resident plan of care and treatment goals related to psychosocial needs
· Schedules and attends care conferences for residents on caseload
· Completes assessments and MDS charting as required
· Serves as a liaison between the resident and the facility. Works with residents and families on concern resolution
· Coordinates resident utilization of community resources
· Works as a member of the interdisciplinary team

Position Type/Expected Hours of Work: Full-Time, 40hrs/week working Monday - Friday
Minimum Qualifications:
· LSW preferred, but not required
· Associate’s degree in social work or related field
· Experience working in long term care or skilled nursing
