Full-time Estimated: $34.7K - $43.9K a year

Obtains medication histories for new patients and documents them in the electronic medical record in a timely manner.
Medication history will be obtained from patient, patient’s family, pharmacy, physician office, or long term care facility. Employee must be diligent to fully explore the medication history.
Communicates with pharmacist appropriately including asking questions, and referring for additional interview or intervention.
Uses good clinical judgement to update the medication history to the best possible list.
Demonstrates knowledge and skills to appropriately communicate and interact with employees, patients, families and visitors of all age groups while being sensitive to their cultural and religious beliefs.

Qualifications:

Education/Skills:

High school diploma or equivalent required; courses in science and math helpful.

Work Experience:

Retail or hospital pharmacy experience required.

Certification/Licensure:

Required

to emergency requests. Ability to assist in meeting the pharmaceutical care needs of neonatal

pediatric, adolescent, adult and geriatric patients. Ability to perform in the conditions of an emergency department.
