POSITION TITLE: Grief and Bereavement Coordinator
REPORTS TO: Director of Mission Services
FLSA: Exempt
Position Scope:
The Grief and Bereavement Counselor, under the direction of the Director of Mission Services provides emotional counseling and support to patients, significant others, families, and other team members. Develops and maintains bereavement support services as indicated for the community and maintains all aspects of the bereavement program.

I. Essential Functions/Performance Factors
· Supervises bereavement activities.
o Oversees all activities related to records of bereavement services.
o Ensures the bereavement team is in compliance with agency guidelines.
· Collaborates with the Director of Mission Services to provide bereavement training programs for Valley Hospice staff.
o Develop and implement bereavement education programs on an as-needed basis for staff.
o Provide bereavement educational programs to the professional community on an as-needed basis.
· Collaborates with the Director of Mission Services to design and implement a community bereavement program.
o Develop and act as the facilitator to community bereavement support groups.
o Coordinate the organization of children’s bereavement programs and facilitate when appropriate.
o Develop program goals and oversee the implementation of activities to meet these goals.
o Participate in therapeutic community support following a crisis, i.e. suicide from a community member.
- Collaborates with the Marketing Director to plan and coordinate the annual Memorial Service held for families.
- Collaborates with the Director of Mission Services and Director of Marketing for the annual Butterfly memorial program..
· When needed assesses pre-bereavement needs of the patient/family at the patient's admission.
o Reviews the grief/loss section on the initial admission form completed by the Social Worker.
o Begins evaluation of family’s bereavement needs at patient’s admission and throughout patient’s stay.
o Evaluates continually effectiveness of intervention and make appropriate changes to Plan of Care.
· When needed assesses the needs of the bereaved; develops, implements, and periodically evaluates a plan of care.
o Completes initial assessment within agency time frames.
o Develops and implements a Plan of Care related to identified bereaved.
o Evaluates continually effectiveness of interventions and make appropriate changes to Plan of Care.
o Assesses needs of any children/teens in family related to grief and loss issues.
· Fulfills speaking engagements in the community when Bereavement information or education is requested.

· Attends continuing education programs as requestedto update the quality of the bereavement program.
· Documents care provided in an accurate, complete, and timely manner.
· Participates in performance improvement activities.
· Uses agency resources wisely.

Supervisor Responsibilities:
Education and Work Experience:
- Master's Degree in Social work or Counseling
- One to two years of related experience
- Evidence of experience and skills necessary to perform job
- Exhibits excellent communication skills
Certifications and Licenses:
Ohio and WV for Counseling
Current driver’s license, automobile insurance, and reliable transportation

WORK ENVIRONMENT
While performing the duties of this job, the employee frequently is exposed to outside weather conditions when traveling between sites. Patients’ home environments are variable with regard to heat, noise, air circulation and conditioning, space, cleanliness, accessibility and all other environmental factors. Inpatient and skilled nursing facility environments are standardized. Office environment in a controlled atmosphere building.

Pay: $45,000.00 - $55,000.00 per year
