Clinical Case Manager - MISSION Program Director


The Clinical Case Manager will provide services to 40 returning citizens from the Lowell/Lawrence Drug Courts and House of Corrections. These services are provided in partnership with Peer Support Specialists designed to support clients in their recovery and transition to living in the community. Clinical Case Managers are responsible for completing the Intake/Assessment, grant specific paperwork including baseline, 6 month, discharge data sets in the Virtual Gateway. In collaboration with the client and peer support specialist create the individual service plan with concrete goals and services to be delivered. Clinical Case Managers will provide group and individual counseling based on the needs of each client these may be delivered in the office or community setting. The team will work cooperatively with the HOC treatment team to help clients succeed. Clinical Case Managers should be familiar available services in Franklin County, and knowledge of the continuum of care for substance abuse treatment.


  • Conduct Bio-Psycho-Social Evaluations to people struggling with substance abuse issues
  • Meet clients at least weekly to provide appropriate clinical treatment
  • Create Person-Centered Treatment Plans
  • Attend Clinical Treatment Team Meetings and Clinical Supervision
  • Case Management as needed
  • Create Discharge Plans


  • Master's degree or LADC I
  • Two years in the field of alcoholism and substance abuse
  • Knowledge of substance abuse and criminal justice system
  • Knowledge of Motivational Interviewing and the Stages of Change
  • Must have effective speaking and writing skills
  • Knowledge of laws governing confidentiality and rules of ethical behavior
  • If in recovery from alcohol/other substances have two year of continuous sobriety
  • Must meet Massachusetts and HOC CORI requirements.
  • Must have car and Mass Drivers License


Mon-Friday 10am-6pm and Fri 9am-5pm (some special events in off hours)
