Position
Description:The Seniors’ Activities Coordinator provides
program coordination and oversight for senior program; this includes but is not
limited to wellness, case management, nutrition, recreation, and socialization
activities.

Essential

Functions:
Senior Advocacy Program

Provide oversight of the senior program, including interacting with the seniors on a daily basis.
Develop and implement schedule of
activities for clients participating in Seniors program
Identify appropriate program responses to
address overarching client needs
Network with community-based and/or
social service agencies to provide additional referral resources to Wesley
House clients
Develop systems to ensure that program
maintains compliance with all contractual and funding obligations
Prepare regular activity reports tracking
program participation and achievement
Submit regular reports to partners
Oversee on-line client activity tracking
system
Coordinate execution of senior
transportation services for participating clients
Facilitate Senior Program Council
meetings

Ensure accuracy of client files and
regularly input client data into database
Attend regular program coordinator
meetings necessary to ensure program compliance
Advocate for needs of clients through
participation in various workgroups/taskforce(s)
Participate in regular professional
development and training activities
Other duties as assigned

Knowledge, Skills,

and Abilities:
Ability to write and speak clearly and informatively; Bilingual, English and
Spanish-speaking required; Strong computer skills including typing and
Microsoft Office; Customer service oriented and able to effectively manage
difficult or emotional client situations; Ability to work cooperatively and
supports the team’s effort to succeed; Ability to interpret and communicate
contractual mandates and guidelines; Strong organizational and project
management skills including planning and scheduling; Understanding of effective
case management strategies and client support resources; Able to lift/carry up
to 30 pounds

Education &
Experience: Associates Degree or at least two years of college required; A minimum of two
years’ work experience in similar organization coordinating program services
and activities, and providing direct case management

Work/Physical Requirements: issues; this will require the
employee to use active listening skills, while also occasionally having to
stand and walk to complete job duties.
