Case Manager

The purpose of the Case Manager is to develop, implement, coordinate, operate, and monitor a pilot Pre-Booking Diversion Program in the Sauk Prairie area for the Criminal Justice Coordinating Council. This is a full-time project position that will be funded until December 31, 2019.

Essential Duties
Develop Pre-Booking Diversion Program.

Develop policies and procedures
Establish screening criteria
Develop forms for evaluation and implementation
Create program goals
Implement, monitor, evaluate, and update Pre-Booking Diversion Program.

Identify appropriate program participants with local law enforcement.

Complete substance-use disorder screenings and assessments to determine the level of services needed.

Perform case management.
Assess each individual
Develop individualized case plans
Refer participants to appropriate treatment to achieve participant goals and case plan
Collaborate and coordinate participant services with medical professionals and treatment providers
Monitor and document progress
Assist with ongoing recovery
Develop re-entry planning following treatment completion
Complete all necessary documentation.
Program forms
Participant records
Case notes
Program data
Program reports
Grant applications and reporting
Provide participant, community, and county education on program and outcomes.
Additional Tasks
Provide transportation to participants for program obligations or treatment.

Provide addiction education and support to participant family and natural supports.

Travel throughout the county and state for:
Participant needs
And other duties as assigned.
Minimum Training and Education
Required: Bachelor's degree in Social Work, Psychology, Human Services, Criminal Justice, or another behavioral science field OR 5-7 years of direct field and office experience in substance abuse

Licenses/Certifications: Wisconsin Substance Abuse Counselor (SAC) or Clinical Substance Abuse Counselor (CSAC)
Complete COMPAS training within 6 months of hire
Valid Wisconsin State Motor Vehicle's License

Preferred: Associate's degree in business or another related field
