Job Description

This position will offer the opportunity to provide patient care with excellent customer skills, right person must be creative with the use of Microsoft software for creative purposes and must be proactive and also able to use own initiative . We are seeking an individual who adequate knowledge on use of Axxess Software, must be a fast learner, attention to details will be every important as you will tested during interview. or willing to be trained. This individual’s primary responsibility will be to coordinate with all parties involved with patient's care and manage the specific clinical services need of the care providers in the field, and must be able to interact regularly with patients and complete additional office administrative tasks when time provides.

Minimum Qualifications:

  • High school or higher
  • Two or more years of related experience


  • Proficient in Microsoft Office with exposure to some data management software experience
  • Experience addressing any patient’s need, must have basic life support certification or ready to trained
  • Experience with working alone and attention to details

Job Requirements:

  • Able to update patient chart and able to write communication effectively
  • Provide communication between agency and the patient primary MD
  • Strong communication skills
  • Explain and answer patient's question or communicate with the charge nurse or doctors caring for this patient
  • Contribute to a positive work environment
  • Act courteously and with tact in dealing with patients, visitors and the public
  • Maintain patient confidentiality
  • Follow established treatment and office procedures
  • Maintain a high degree of accuracy regarding all details
  • Manage referral reporting and marketing programs
  • Physically able to walk, stand, stoop, and bend
  • Visual and auditory dexterity
