Summary: Responsible for providing referral sources with information on palliative care and hospice programs and services and the coordination of patient care for optimal transition from facilities to the hospice care setting.
Education: Bachelor's Degree in Nursing preferred or Graduate of Diploma or Associate Degree program in Nursing.
Experience: Minimum 3 years clinical experience with 1 year in home health care, preferably hospice.
Liaison will cover Boston Hospitals.
