How You'll Make a Difference:
Works with Homecare/Hospice Managers and Field Caregivers to efficiently schedule Homecare Admissions and Evaluations. Acts as a Liaison between the Patient and Field Caregivers.
Completes initial Welcome Call to patient/representative regarding ordered Homecare or Hospice Services. Uses call script to ensure the communication with the patient/representative is thorough per Agency guidelines. Completes Medicare Secondary Payer Questionnaire (MSPQ) during initial call as appropriate.
Schedules and maintains calendar of appointments, meetings, and travel.
Schedules patient appointments and coordinates cancellations and reschedules as necessary. Accurately follows defined Agency scheduling workflows. Communicates closely with the Supervisor/Team Manager/Lead to resolve scheduling issues.
Answers telephone, screens calls and takes messages.
Completes assigned Epic reports per defined timelines.
Generates basic reports, modifies/updates existing reports, and distributes reports as needed.
Mails printed information or supplies to patients or Field Caregivers as requested.
Confirms patient demographic information and may collect insurance info.
Assists with gathering, organizing, and compiling data for reports.
Operates and maintains office equipment (fax, printer, photocopier, etc.) and may order office supplies.

Knowledge, Skills & Abilities
Excellent Communication (written and verbal) and interpersonal skills: ability to effectively communicate with a variety of patients, staff and physicians. Intermediate computer skills including experience in using personal computers: including Microsoft Office or similar application, and electronic mail.Excellent organizational skillsAbility to read and understand verbal and written instructions. Ability to work in a fast paced environment with a strong attention to detail and accuracy.
Monday - Friday 7 a.m. - 3:30 p.m.
Holiday rotation one per year.
