Job Summary:
The Social Worker-assists patients and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from health and medical care. Plans, organizes and implements social work services to patients and families. Provides emotional care, advocacy, and maintains effective working relationships with community resources and services. Reports to Hospice Director. Works with the Bereavement Coordinator on assisting with Bereavement visits. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. . Participates in in-service programs and CEU’s as applicable. Prepares documentation of patient care in the home at the time of visit as applicable and submits documentation in a timely manner according to policy. Notifies health care team of any changes and assist patient and family in meeting needs. Performs other job duties as assigned.
Performance Expectations:
- Demonstrate ability to use specific software to perform pre-certification on patients for insurance eligibilities.
- Ability to perform accurately and timely.
- Demonstrate ability to enter all patients charting in EMR documentation meeting state and federal regulations.
Qualifications:
Education/Skills
Master's degree in social work from accredited school of social work required
Work Experience:
One year of social work experience in a health care setting
Certification/Licensure-DUE UPON HIRE
- Licensed Social Worker (LSW) as defined by Sec. 75-53-1 of Mississippi Code of 1972, required
- LCSW and ACSW (Academy of Certified Social Worker's) certification preferred.
- Valid Driver's License and proof of automobile insurance required.
