We are currently seeking a RN Minimum Data Set (MDS) Coordinator to work out of our office in Bangor or out of our community in Belfast.
Job Summary:
The MDS Coordinator participates in the assessment process by completing information on the Minimum Data Set (MDS). As a Registered Nurse, will care for ill, injured, or convalescing patients in an assisted living residence.
Duties/Responsibilities:
- Manage processing and tracking of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for services provided at facility.
- Conduct MDS reviews to assure achievement of maximum allowable RUG categories.
- Create Point Click Care (PCC) patient profile
- Ensure validation of New Residents by submitting the MDS to Muskie School
- Submit Section 2 weighted score reports to Muskie School
- Recertify each MDS every 6 months
- Track resident cost of care responsibility
- Work with team to complete new assessments every 180 days
- Billing from monthly MaineCare reimbursement invoices
- Enter rent collection per METPID
- Complete RN quarterly assessments, or with change in resident conditions.
- Oversee the care of all Maine residents through oversight, teaching, and training staff
- Ensure resident files are prepared for survey
- Assist to complete POC (plans of correction)
- Complete service plans within established time frame for all residents
- Ensure proper and timely communication with MDS Coordinators to meet established regulations.
- Collaborative role with human resources, recruitment, retention of new staff through training and evaluation.
- Identify and report areas of improvement opportunities.
- Work with RN Care Coordinator on division of duties.
- Other duties as assigned by the Regional VP of Maine.
Required Skills/Abilities:
- Experience working as an RN in an Assisted Living environment, preferred.
- Knowledgeable in State and Federal regulations.
- Must have valid driver’s license and reliable transportation.
- Excellent verbal and written communication skills.
- Excellent interpersonal and customer service skills.
- Excellent organizational skills and attention to detail.
- Excellent time management skills with a proven ability to meet deadlines.
- Proficient with Microsoft Office Suite, PCC or related software.
- Willingness to travel to assigned Communities, as needed.
Education and Experience:
- Licensed Registered Nurse or LPN in the State of Maine.
- At least one year experience working as a MDS Coordinator.
- Attended MDS training through the State of Maine.
Physical Requirements:
- Prolonged periods sitting at a desk and working on a computer.
- Must be able to lift up to 25 pounds at time.
- May require prolonged periods of standing, moving and bending.
