Wellness Director Position Summary

We have a remarkable opportunity for an exceptional seasoned Wellness Director to take the helm of our large and growing Assisted Living and Memory Care Units in our beautiful CCRC in Central Phoenix.

We are searching for an outstanding individual with a unique mix of senior-based nursing knowledge that includes both Assisted Living and Memory Care who is an accomplished leader, focused on Customer Service and enjoys the elderly. This fun and exciting CCRC sits on 13 acres and is comprised of 400 Units. This Phoenix property offers its residents the true continuum of care with Independent Living Apartments, Assisted Living Apartments, Memory Care Units and a 60 Bed 5-Star Skilled Unit.

Here’s the BEST part…we are in the middle of a 10 Million Dollar renovation and will soon be the most opulent and grand CCRC in town. In addition, we are opening a NEW 170 Unit Assisted Living in early 2019 which boasts amazing neighborhoods and some of the largest fully renovated AL Apartments with access to our incredible Main Street which will open in April.

The Wellness Director/Director of Community Care supports the Executive Director in achieving the goals and objectives for the community by ensuring high quality clinical services at the community. The Wellness Director partners with Palazzo Executives in order to execute policies and procedures according to company standards and applicable local, state, and federal guidelines. Actively participates in the delivery of care services and medication administration as needed. Manages clinical care budget through effective staffing and cost control and takes an active role in the admissions assessment process.

The Wellness Director will assemble their own team, in part to compliment the particular skills and knowledge he or she brings to the task. He/she must possess a high level of energy and a true passion for quality care, community development and community revitalization.

This critical leadership role will oversee the overall day-to-day operations of the AL/ALZ while maintaining compliance with all Arizona State and Federal Regulations: Hiring, training and supervision of qualified staff, ensuring adequate staffing while maintaining continuity and consistency in delivery and quality of services; establishing a working relationship with regulatory agencies, family members, and community advocates; quality assurance; participating directly in sales and marketing while helping to maintain a high occupancy through the creation of stellar quality of care. You must have a proven ability to work with residents and their families to achieve the balance of quality of life and business objectives.

Responsibilities:

  • Manage the day-to-day healthcare operations of this AL/ALZ Community to ensure residents’ healthcare needs are met
  • Provide overall clinical leadership
  • Coordinate nursing/resident services to ensure the continuity of the resident’s total regimen of care
  • Supervise all care staff, including interviewing, evaluating, training, mentoring, assigning work schedules, developing performance improvement plans
  • Participate in the development, implementation and enforcement of policies, procedures, and systems
  • Assure regulatory compliance at all times
  • Participate in Quality Assurance Program
  • Adhere to the department’s budgetary obligations
  • Communicate with physicians and family members
  • Resolve complaints regarding resident care
  • Provide training and supervision to associates in the administration of direct care within the community; provide hands-on resident care as needed or warranted
  • Conduct quarterly assessments and develop subsequent service plans
  • Perform ongoing nursing assessments/observations of residents’ physical and psycho-social needs, and coordinate with other departments to assure quality care
  • Provide continuity of care for those residents receiving home health care, hospice services, and other third party healthcare-related services
  • Serve as a point of contact for the resident’s family members, vendors and professional contacts in the community, dealing with issues such as scheduling, resident care, service plans, assessments and marketing

Qualifications/Knowledge/Skills/Abilities:

  • Associate’s or Bachelor’s degree in Nursing strongly preferred
  • Active RN or LPN licensure in the State of Arizona. Must be in good standing
  • Minimum of five years of progressive Nurse Management experience required including experience working with dementia or Alzheimer’s residents.
  • Demonstrated working knowledge and experience with Geriatric Nursing and care services for a wide variety of residents
  • Experience opening a new Assisted Living and/or going through the State licensing process a plus
  • Ability to effectively communicate with residents, their families, and with staff at all levels of the organization.
  • Strong interpersonal skills with the ability to communicate effectively to a wide range of audiences, both internal (care staff, nurses, executives, etc.) and external (families, residents, physicians, vendors, etc.)
  • As in any nurse management job just be able to work occasional nights/weekends and holidays as required and to be on call via telephone for nursing related emergencies.
  • Current First Aid and CPR certifications
