- Position: CCA Coordinator
- Pay Range: $21-25/hr
- Reporting To: CCA Manager

POSITION SUMMARY
The Assisted Living Waiver Program Care Coordination Agency (ALWP CCA) Office Coordinator is responsible for processing the ALWP beneficiary application as per Regulatory Agency and Libertana Home Health Policy.
QUALIFICATIONS
- Knowledge of the State of California Assisted Living Waiver Program Care Coordination Agency Provider Handbook, RCFE Provider Handbook and Title 22 Division 6 for RCFE.
- Exercises excellent judgment and has good organizational skills.
- Fluent in English. Possesses effective written and verbal communication skills.
- Proficient in the use of computers, electronic medical records, and Microsoft Office.
- Ability to establish and maintain good communication and relationships with office, field, and administrative personnel.
- Is self-directed with the ability to work with little supervision.
- Must be a licensed driver with an automobile that is insured and is in good working order, in accordance with state and/or organizational requirements.
- Preferably, has a Bachelor’s degree in health care/business administration or a related filed.
- Previous Health Care experience preferred.
- Background and/or knowledge of developing reports, newsletters, statistics, and information analysis desired.
- Possesses excellent analytical skills.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Assists Medi-Cal beneficiaries in obtaining access to waiver services, state plan services and other community resources by identifying, coordinating and collecting data required for the ALWP application.
- Responsible for the proper completion of all documents required for the State Waiver Program Application as per ALWP and Agency policy.
- Reviews and evaluates the ALWP documents necessary for State Waiver Program Application, including Initial Assessments, Re-assessments, ISPs, Physician’s Reports Medi-Cal eligibility, etc.
- Discusses State inquiries or requests for documentation with CCA Manager, CCA Staff, SNFs and other referral sources
- Responsible for the completion of Patient Charts as per Agency policy.
- Establishes and maintains good relationships with RCFE, SNFs and other vendors.
- Assists the Agency in maintaining compliance with Federal, State, Local and HIPAA Regulations or Joint Commission Standards.
- Oversight and Process Improvement: You will provide oversight for the CCA Manager, ensuring efficient and effective operations within the department. Working collaboratively with other team members, you will identify discrepancies in processes/procedures seeking opportunities for improvement to enhance overall department performance. Will recommend process/procedure changes to CCA Manager for Care Coordinators.
- Providing Expertise in Complex Client Issues: Your support and expertise will be crucial in solving client issues, demonstrating your ability to navigate through challenging situations while ensuring the highest level of client satisfaction and compliance with ALWP. Involves CCA Manager as needed.
- Oversee a team of 3-5 Care Coordinators and ensure that Job Responsibilities are met.
- Maintain a small caseload and assist client with ALWP services.
- Assists in assigning caseloads/clients for Care coordinators
- Assist Care Coordinators, RCFE managers and contacts to verify ALWP billable clients. Involve CCA Manager as needed. Assist Care Coordinators in compiling and reconciling ALW monthly census. Assist Care Coordinator in resolving identified discrepancies.
- Assures that assessments are performed and all necessary documents are completed.
- Troubleshoots Client, RCFE, SNF and other vendor issues. Follow up with RCFE managers, discharge planners and other appropriate client contacts who work there, establishing working relationships and educating them about the ALW Program the services available to them
- Assist CCA Manager in releasing and monitoring the DHCS ALW Waitlist Releases to ensure proper record keeping and timely submission.
- Collaborative Interdisciplinary Work: You will work collaboratively and interdisciplinary with the entire CCA team as well as others outside of the department and/or company, fostering a cohesive and inclusive work environment.
- Assist Care Coordinators and CCA Supervisor in providing ALWP in-services to clients.
- Assist Care Coordinators in providing necessary documentation needed in the event of a DHCS Facility Audit.
- Provides Care Coordinators with Training Refreshers weekly based on performance, trends, and updates that are identified.
- Meet weekly with Care Coordination Team to ensure that time sensitive deadlines are met to maintain beneficiary enrollment in ALWP. Provides oversight, and training in ensuring all necessary documentation is in an organized fashion in all department platforms
- Oversees proper tracking, charting, progress notes, and case records for assigned Care Coordinators.
- Training/Precepting new-hires.
- Assists facilities and Care Coordinators in Habilitation Hours compliance.
- Exercises excellent judgement and knowledge of current Care Coordination practices and procedures in alignment with Libertana Policy and DHCS ALWP Policy
- Participates in appropriate continuing education as may be requires and/or required by your immediate supervisor.
- Maintains a comfortable work environment for all employees.
- Conforms to all agency policies and procedures.
- Respects the patient’s and family’s rights and property as defined by the federal and state laws.
- Support CCA Manager in the completion of annual reviews of Care Coordinators.
- Some driving may be required around Los Angeles, San Bernardino, Bakersfield Riverside, and San Diego County.
- Knowledge of Confidentiality, HIPAA and healthcare laws and regulations.
- Knowledge of mandated reporting
