Outpatient Therapist - $500 SIGN ON BONUS
Scope:
In collaboration with the Director of Outpatient and Partial Program, the Outpatient Therapist (OPT) is responsible for individual and family therapy provided to consumers on their caseload. This includes weekly individual therapy and family therapy as recommended or necessary. Caseload size varies based on available cases and the OPT's schedule. OPTs manage their own appointments and communicate with families/consumers regarding appointments.
Documentation Responsibilities:
- Maintain encounter forms and GDAP notes for each therapy session.
- Update treatment plans every 90 days or 10 sessions.
- Complete discharge summaries and aftercare plans upon completion of a consumer's treatment.
Communication:
- Maintain therapy appointments, following up on cancellations and no-shows.
- Obtain necessary signatures from consumers and their guardians on paperwork.
- Report the need for correspondence regarding continuing treatment to the outpatient supervisor weekly.
- Report the need for psychiatric/psychological updates weekly to the outpatient supervisor.
Direct Care:
- Provide child-centered, family-focused individual and family therapy.
- Intervene during crises and oversee CRC transportation and accompaniment if necessary.
Documentation:
- Complete GDAP notes and encounter forms weekly for each therapy session.
- Document clinical progress for missed therapy appointments on a weekly basis.
- Update treatment plans reflecting current goals and progress every 90 days or 10 sessions.
- Complete discharge summaries, discharge notes, and aftercare plans within one month of a consumer's discharge.
- Document as-needed telephone contacts with consumers and their families.
Planning:
- Schedule meetings with consumer guardians to complete treatment plans.
- Maintain a weekly therapy schedule for their caseload as assigned by the Director of Outpatient & Partial.
- Assist supervisor in helping consumers attend scheduled doctor’s appointments.
Trauma-Informed Behavior:
- Willingness to learn and maintain the value of a trauma-informed culture.
- Commit to the maintenance of a therapeutic environment and community that promotes safety, nonviolence, and resilience as the basis for all we do with clients and staff.
