Manager-Care Coordination (Inpatient)
Responsible for the oversight of daily operations, staffing, and workflows for the Care Management Department. In collaboration with the hospital-based leader, this professional manages a team of professional staff, composed primarily of nurses and social workers. This team is responsible for providing care management services to the hospital's patient population that consists of care coordination, care progression, discharge planning, medical necessity reviews, and psycho-social services. This team also works in collaboration with the medical and nursing staff to manage length of stay and the care of patients at risk for readmission. Under the Director's guidance, the Manager leads the team in continuous process improvement activities to ensure that patients receive cost-effective, high quality, and safe care. This professional also collaborates with both hospital and care management leadership to create and achieve department goals and initiatives. This includes supporting the tracking, trending, and reporting of department metrics that reflect the hospital's financial performance and progress toward patient satisfaction goals. As a member of the leadership team, the incumbent serves as a role model that supports professionalism, service, positive communication, teamwork and safety.
Operations management, customer/peer relations, quality assurance, education, clinical practice and leadership.
Education/Knowledge: Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline. Education Discipline: Bachelor of Science in Nursing (BSN). Experience: 3-5 years
Maryland Registered Nurse License (RN)
CCM or ACM preferred, or certification within 3 years of hire.
