Full Job Description
The Intake Coordinator is a critical member of the Behavioral Health department of Westmed Medical Group. The Intake Coordinator is most often the first person to help patients families connect with our services. Our environment requires an energetic and caring/empathetic individual who has the ability to multitask and communicate effectively with patients and colleagues. Primary focus will be to support the initial assessment and evaluation of patients to determine behavioral health needs and establish initial care requirements of new patients. Will assist Associate Medical Director and providers in the creation and implementation of best practices. Relies on extensive experience and judgment to plan and accomplish goals.
Essential Functions and Job Responsibilities:
To oversee intake process; evaluation and triage of new cases
To assist the Associate Medical Director, (“AMD”) with liaison activities; setting up regular meetings, provide curbside consults, as needed/required.
Serve as a liaison with the community in order to set up referral networks and be involved in educational activities associate with WESTMED Behavioral Health.
Work with the Behavioral Health Team and AMD on program development, case management, referral process.
Collaborates with the patient experience associates and Patient Experience Manager with specific regard to the referral/intake process and help with insurance authorizations/treatment plans and tracking of patient visits.
Works on and organizes Quality Assuranceprojects, analystics, and the implementation of screening instruments.
Assists clinicians with case management, drug authorization forms, disability paperwork
Other duties as assigned by management, department providers, and the AMD.
Is a strong ambassador of WESTMED Medical Group and demonstrates high levels of compassion and empathy towards the patient experience.
BS or MS in the Mental Health Field
At least 2 years’ experience working in the mental health field
Experience with community outreach
Experience with intakes/evaluations process
Excellent interpersonal skills and experience working in a team based clinical environment.
