The Treatment Options Educator develops, implements and provides treatment options education to CKD (chronic kidney disease) patients, Nephrology office staff, Hospital Case Managers, Hospital Discharge Planners and USRC Staff.
Provide treatment option education to all new patients admitted to in-center facilities within the first 2 weeks of admission.
Strategically promotes USRC Treatment Options Program to Nephrology offices, Hospital Case Managers and Primary Care Physicians.
Establish process for multiple disciplines to refer patients for Treatment Options Education.
Develop and provide scheduled Treatment Options class for Stage 3 & 4 CKD patients and their families.
Provide assistance and education to Nephrology office staff on identification of Stage 3 & 4 patients. Participate in USRC staff training and education programs t ensure knowledge of program.
Maintains patient contact information and status; reports weekly progress to National Director of Home Therapies.
Cultivate professional relationships outside of USRC by participating in community activities.
R.N. with valid licensure in the applicable state is preferred.
At least three (3) years of experience in Nephrology nursing.
Strong customer service skills.
Experience in educating patients and staff.
Must be competent in MS Office Word, Excel, PowerPoint and Outlook.
Must have excellent presentation skills
Must have a valid driver's license with reliable transportation and proof of current auto insurance.
Bilingual, Spanish speaking is a plus.
