Graduated from an accredited school of professional nursing.
Bachelor's Degree in Nursing required.
Strong demonstrated clinical knowledge and skills; good oral, written and interpersonal skills, strong analytical and organization skills.
Current state licensure as Registered Nurse (RN).
Proof of Basic Life Support (BLS) certification must be presented upon hire.
Valid Ohio Driver's license
Must have reliable transportation and proof of automobile insurance coverage with $100,000/$3000,000 coverage
Minimum of three year's clinical experience; three years in home health setting highly preferred.

Provides direction and support to the home care field staff.
Supervises start of care, resumption of care and routine visits.
Conducts assessment of patient's needs and initiates nursing care plans.
Evaluates and documents patient progress.
Facilitates the planning, implementation, development, and evaluation of nursing programs, initiatives and objectives for the geographic area.
Assists with the development and implementation of policies and procedures to insure efficient and effective delivery of health services in a home care and serves as a liaison between the Director of Nursing, staff and other areas within the organization.
Identifies and resolves issues affecting the delivery of patient care services in the homecare setting.
Oversees nursing personnel and patient care providers to ensure quality of care. Anticipates and resolves patient care issues.
Ensures continuity of patient care by actively participating in and facilitating communication and collaboration among and between department staff and other departments re: operational issues.
Reviews major operational patient care issues with director of nursing and provides ongoing clinical evaluation of nurses and other direct and indirect patient care providers in assigned area.
Counsels employees on performance issues, administers initial corrective actions, collaborates in monitoring attendance issues.
Alerts director of nursing of significant clinical or other performance issues.
Participates in interviewing, selecting and appraising the performance of employees.
Monitors the adherence to EEO/AAP goals and guidelines.
Administers nursing institute, departmental and CC policies and procedures.
Monitors operations of outpatient clinics (patient flow, staffing, supplies, etc.).
Prepares schedule of work assignments assuring adequate coverage on all shifts.
Participates in and supports quality improvement projects, outcomes studies and research initiatives.
Participates in department and committees as assigned.
Assists in orienting new personnel, continuing education and in-service educational programs.
Other duties as assigned.
Requires full range of body motion including handling and lifting patient, manual and finger dexterity, and eye-hand coordination.
Requires standing and walking for extended periods of time.
Requires corrected vision and hearing to normal range.
Requires working under stressful conditions and irregular hours.
Exposure to communicable diseases and/or body fluids.
Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
Physical Demand requirements are in excess of those for Light Work.
