We are looking for a dynamic and experienced professional to join our team as a Case Coordinator in Hartford City.
Case Coordinators will provide vital support and comprehensive case management to individuals and families in Blackford County. In this role, you will make a direct impact on the lives of those in our community by conducting assessments, developing personalized service plans, coordinating care, and advocating for clients' access to essential community resources.
You will work closely with clients, families, community organizations, and service providers to promote well-being, self-sufficiency, and active community engagement.
MINIMUM QUALIFICATIONS
- Bachelor’s degree in social work, psychology, counseling, or a related field.
- Minimum of 2 years of experience in crisis intervention, case management, or a related field.
- Experience working in rapid response or emergency services preferred.
- Relevant licensure or certification preferred.
- Valid driver’s license and reliable transportation for community-based work.
- Ability to work flexible hours, including evenings, weekends, and on-call shifts as needed.
- CPR and First Aid certification preferred.
