Position Summary

La Paloma Treatment Center is currently seeking an Intake Coordinator to initiate and complete the referral and intake process, determine appropriate level of care and maintain follow-up communication with referral sources, families, patients and program staff to maintain a quality intake. We are looking for someone with the ability to consistently demonstrate through conduct and communications the stability required to effectively assist patients and their family members through the FRN Model of Treatment.

Essential Duties and Responsibilities

Assesses perspective patients for treatment, identifying psychosocial, mental health and medical issues and completes the clinical psychosocial assessment to determine whether or not appropriate for program and if so, then determines treatment level.
Completes Utilization Review Pre-Certifications.
Works collaboratively with the Admissions Center, Utilization Review, Nursing and Residential Counselors to facilitate an efficient transition of patient into the treatment milieu.
Attends staff meetings and treatment team meetings to monitor results of patients and participates in clinical treatment planning.
Arranges appropriate transportation services as needed to transport patients.
Responsible for introducing new patients to Therapeutic Community.
Explains the evaluation and admission process to the potential patient/referral source and informs patient of necessary items to bring for treatment.
Documents all intake information into the TIER program including demographics and all appropriate assessments.
Maintains daily census by reviewing bed boards to determine appropriate number of beds available.
Creates intake file ensuring that the intake paperwork is complete and accurate and files intake-related documents appropriately.
Maintains professional relations and assures information is communicated effectively with staff as well as to patients, families and referral sources.
Performs all other duties as required or assigned.

Knowledge, Skills and Abilities

Ability to inspire patient confidence in The Oaks staff and the System of Care.
Adhere to the JCAHO standards.
Ability to consistently demonstrate through conduct and communications the stability required to effectively assist patients and their family members through The Oaks System of Care.
Excellent verbal and written communication skills required.
Must have high level of interpersonal skills.Position continually requires demonstrated poise, professionalism, tact and diplomacy.
Exercise good judgment, apply common sense and successfully handle ambiguity.
Understanding of diverse cultures and gender specific issues and the ability to incorporate needs of gender and culturally diverse groups into the organization.
Excellent organizational skills with the ability to prioritize workload and meet deadlines.
Must have strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, PowerPoint).Proficiency in MS Access (or other database program) is preferred.

Understands need for and maintains appropriate confidentiality at all times when interacting with patients, residents, families, visitors, referral sources and all other contacts.
Exhibits excellent customer relation skills as evidenced by supportive and constructive communication with all contacts including coworkers, patients, residents, visitors, families and referral sources.
Knowledge of addiction, addictive process and recovery.
Knowledge of group process, group facilitation, and conflict resolution.

Education and Experience

Master’s degree required in psychology, social work, or related field and working toward Clinical License; RN or Clinical License preferred.
Minimum 1 year experience working in a psychiatric/substance abuse facility required.
