Job Summary
The Licensed Vocational Nurse (LVN) functions under the direct supervision of the Nurse Case Manager (RN). The LVN provides nursing care to patients of all ages; from pediatric to geriatric, within the LVN scope of practices – as delegated by the Nurse Case Manager or other advanced nursing staff. The LVN may practice in any of three clinical areas: in an office/ clinic, providing care for onsite recuperative care patients, and as an integral part of the outreach street medicine team providing care to unsheltered homeless patients.
Responsibilities:
- Provides nursing care for all patients within scope of LVN practice
- Assumes responsibility and accountability for designated patients
- Provides nursing care to patients of all ages: children, adolescents, adults, and geriatric
- Gathers data relevant to the patient’s individual needs and age group
- Documents nursing observations, treatments, and tolerance according to charting standards
- Administers medications and treatments per clinic policy
- Reports symptoms, reactions, and progress of patient to the Nurse Case Manager or advanced practice nurses.
- Pursues proper chain of command to attain a satisfactory solution for difficult to accomplish patient care needs
- Initiates, with the assistance of the Nurse Case Manager, changes to protocol(s) to improve patient care; discusses changes with Nurse Case Manager and advanced practice nurses as needed
- Utilizes educational opportunities within the clinic and outside organizations to maintain clinical expertise and to promote personal growth and development
- Responsible for knowledge of legal aspects of nursing
- Ability to work 7PM-7AM shift as needed, if scheduled in recuperative care section
- Furthers his/her education by attending in-service classes, staff meetings, continuing education classes and reading of current professional material
Qualifications
- At the time of application have a current and unrestricted LVN in the state of CA
- Must present a professional demeanor
- Must have good interpersonal skills
- Must maintain a calm and reasonable attitude during crisis and conflict
- Must demonstrate initiative and good judgment
- Must respond appropriately to routine and emergency situations
- Must be able to set and revise priorities based on changes in the situation
- Highly desired: experience with underserved populations facing socioeconomic barriers to health care
- Highly desired: fluency in Spanish
- Preferred 1-2 years of experience as an LVN in an acute care hospital or community clinic setting
- Current unrestricted driver's license
- Current Basic Life Support for Healthcare Professional certification
- MUST pass a background check
Job Types: Full-time, Per diem
Pay: $27.00 - $29.00 per hour
