Facilitate Early Intervention Services in Northeast Colorado. This includes Logan, Morgan, Yuma and Sedgwick Counties. Full time- 35 hours/week
Responsibilities and Duties
Facilitate family applications for services including services offered by ECSDD. Coordinate the performance of evaluations and assessments and assure that eligibility is determined according to the State Plan. Facilitate and participate in the development, review and evaluation of the Individualized Family Service Plan, assuring that family priorities are being addressed. Perform monitoring activities to ensure that the child and family are receiving services and supports as outlined in the plan. Coordinate services with other agencies as needed.
Qualifications and Skills
B.A. or B.S. in Child Development or other related field
Must possess excellent writing, organizational, and verbal skills and the ability to communicate effectively in order to work in a team oriented environment.
