Job Purpose: Assist in the coordination of services for the day support program. Provide supervision, training and guidance to program participants. Assist with the development and implementation of behavioral and vocational plans. Ensure compliance with and all regulatory bodies (Medicaid, licensure, CARF, etc.) as well as, other funding sources and company policy and procedures.
Be knowledgeable about each participant's program in the designated group. Implement and initiate active participation in the daily operations of the day support program. Follow outcomes/strategies/protocols, etc as written.
Maintain active involvement in all daily group and individual areas, including, but not limited to community integration, training, hygiene, leisure, vocational, social, communication, mobility, behavior, toileting, food preparation and feeding, assistance with bus loading/unloading, medication, lunch supervision, etc.
Maintain accurate records and files according to regulatory and company standards. May include maintaining case records according to CARF standards. Maintain knowledge on programs and medical information on all participants. Notify the Program Manager of missing or inaccurate information immediately.
Communicate with parents, guardians, residential counselors regarding participant illness incidents, etc., at the direction of the Program Manager or Assistant Program Manager. Apprise the Program Manager and team members of any concerns as needed.
Promote a safe, clean, and orderly work environment. Follow all established written protocols, policies/procedures regarding safety and risk management. Report any concerns immediately to proper management.
Additional Duties and Responsibilities: Utilize appropriate behavior management techniques.Participate in department, consumer, and staff committee meetings.Use proper lifting, feeding, and transferring techniques.Other related duties as assigned.
Education, Experience, Certifications and Licensures: High school diploma and 2 years experience working with adults with developmental disabilities or in related field.
Knowledge, Skills and Abilities: Relates well to people with disabilities.Physical ability to assist participants with toileting, mobility, transferring, etc. as well as lifting materials and equipment up to 30 pounds.Employees who drive CFS participants in the community must be eligible and covered under CFS driver's insurance.
Physical Demands: While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel and talk or hear. The employee frequently is required to sit and reach with hands and arms. The employee is occasionally required to stand; walk; climb or balance; and stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 30 pounds and occasionally lift/move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
Work Environment: While performing the duties of this job, the employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually community volume
