The Doctor’s Choice Home Health Agency is currently seeking experienced and dedicated Licensed Vocational Nurses to join our Home Health team at our Riverside office. The skilled nurse will be able to make visits within (10-30 miles distance) from their home to patient’s place of residence, in order to provide skilled care services as ordered by referring physician.
**Must Be Able to Cover Hesperia, Victorville, and Apple Valley** (Adelanto and Phelan are also available to cover, but not required)
Per-Diem status only (Skilled Nurses are able to choose areas of their choice).
Minimum Requirements include:
- Graduate of a National League for Nursing accredited school of nursing
- Must have minimum 1 year of experience as an LVN field nurse in Home Health
- Be available to work at least 3-4 days a week
- Experience in EMR software (preferred).
- Provide skilled nursing services / interventions in accordance with physician orders.
- Assesses patient needs, plans for, evaluates and modifies care to meet goals of skilled nursing interventions.
- Collaborates with all disciplines to plan and evaluate team goals for each patient.
- Wound Care experience
Required Licenses and Certifications
- LVN license in CA
- Current CPR certification
- Valid Driver’s License & Auto Insurance
- Physical and TB from the last 6 months or sooner
Pay: $40.00 - $50.00 per hour
