We are currently searching for our next Case Manager for our Teen Parent Support Program in Southbridge, MA!

Case Managers are responsible for providing an enriching and supportive physical and emotional environment for pregnant and parenting youth aged 24 or younger, at-risk youth, and other diverse populations.
Persons in this position will be responsible for meeting with clients according to directions from the supervisor and the treatment plan.
The Case Manager will act as a model and advocate for the client and will engage in activities with the client to bring about changes in behaviors and facilitate stable community living.
Case Managers are responsible for helping clients in creating a broad social network and teaching the client to make use of those supports.
The primary responsibility will be to assist the client in building the skills necessary to reach their maximum potential in society.
Carry an average of 15 clients, age 24 or younger, who are pregnant or parenting or at high-risk for teen pregnancy.
Teach clients to effectively access community resources (Housing, Employment, Health Care Food Banks, Public Transportation, Parenting Groups, Advocacy groups, etc.)
Help clients with household management (budget, menu planning, family rules, chore lists, sticker charts, recreational activities etc.)
The schedule for this position is Monday-Friday.

Candidate must be 21 years of age.
Candidate must have a valid driver's license and reliable transportation.
It is strongly preferred that candidates have experience working with at risk youth, biological families, adults with intellectual disabilities, and other diverse populations, including coaching activities within the community required.
BA/BS in human services, social work, counseling, psychology, or related field, plus at least six months of full-time experience working with at risk youth, clients with significant emotional, mental, and physical needs in many different settings, or related experience is required.
Related clinical experience working with children, adolescents and families required.
Ability to be flexible, and able to change direction rapidly.
Ability to work independently, prioritize workload, complete a wide variety of tasks in a timely and accurate fashion and direct others, as needed.
Candidate must have experience using Microsoft Suite, including: Word, Excel, Power Point, etc., PDF and Outlook , and willingness to be trained in CareLogic.
Bilingual English/Spanish is preferred.
