Overview:
Crescent Hospice is currently hiring Full Time Compassionate On-Call Registered Nurses (RN) to provide patient care in and around the Murrell's Inlet Area!
The On-Call RN is responsible for identifying and coordinating patient/family needs and for providing preventative and supportive care to the hospice patient and family. The On-Call RN utilizes assessment, intervention, and teaching skills to maximize patient comfort and to enhance the quality of life. The On-Call RN assumes on-call coverage for the designated teams and functions as an Interdisciplinary (IDT) team member to provide hospice care, support , and interventions after hours.

Responsibilities:
- Provides emergency, PRN, routine, and admission visits as necessary or scheduled.

Assumes primary responsibility for hospice patients during on-call hours.

Provides ongoing assessments of physical, psychological, and social and spiritual needs of the hospice patient and family and initiates appropriate interventions.

Reviews clinical referral information and responds to requests and inquiries as appropriate Collaborates with Administrator On-Call regarding the eligibility and appropriateness of client for hospice services.

Assesses and evaluates patient's status by: following the plan of care, regularly reevaluating patient and family/caregiver needs, participating in revising the plan of care as necessary based on patient status and needs.

Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing interventions/actions. Includes the patient and the family in the planning process.
Qualifications:
- Bachelor's degree in nursing or health care preferred.

Hold current unencumbered license as a Registered Nurse.
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting.

Hospice/home health experience preferred but not required.

Must be computer proficient in typing and various programs, including background in EMR.

Possess and maintains current CPR certification.

CMS requires that all patient-facing staff, as well as those that are in contact with patient-facing staff, be fully vaccinated, or have a documented religious or medical exemption. This important rule is required for all healthcare organizations that receive federal funds through Medicare or Medicaid.
