Summary: Under general supervision, conduct culturally appropriate health education, case coordination & management, and community outreach alongside the HIV Prevention team. Provide HIV prevention information within the community to reach target and underserved populations and assist them to adopt healthy behaviors. Provide intensive care coordination, support, and services to HIV-negative individuals who require assistance in accessing and remaining in PrEP care. Conducts outreach events in relevant communities, pre- and post- test client centered counseling, rapid HIV testing, risk assessments and risk reduction coaching. This position works to promote and protect the health and safety of the residents of Cobb and Douglas counties.
Minimum Qualifications: Bachelor’s degree in social services related field from an accredited college or university AND one year of experience in a social services related field OR High School Diploma or GED AND 3 years of experience in a social services related position OR 1 year experience at the lower level Social Services Tech 2 (SST011) or position equivalent.
Preferred Qualifications/Skills: Experience with HIV prevention activities. Excellent verbal and written communication skills.
Work Conditions & Physical Demands: This position does require some physical exertion. May require some lifting (no more than 50 lbs.) Requires flexibility of working in general office environment, clinic environment and community outreach. Will require working directly with blood borne pathogens.
Salary Information: $26,010 Annually / Pay Grade F
