Masters in a mental health related field preferred. Previous experience providing assessment and case management services to the homeless population preferred.
A minimum of 2 years of experience working with the mentally ill, substance use and homeless population preferred. Provide care coordination, case management and assessment services in the North San Diego region.
Strong written and verbal communication skills.
Able to work independently as well as within a team.
Clean driving record, current CA driver's license. Must be 21 or over to drive.
Builds and maintains collaborative relationships with hospitals, primary care, substance use, mental health programs and other community referral sources.
Works with Peer Specialists to promote engagement and linkage to services in the community.
Provide comprehensive case management services as a Case Manager / Social Worker and facilitate needed services.
Obtain benefits such as SSI, Medi-Cal, GR, EBT. Perform as an advocate for program participants.
