RN needed for this beautiful long term care facility. MDS/Care Plan Coordinator position duties include timely and accurate evaluation of resident needs, coordination of patient care services, monitoring outcomes and development of the individualized care plan. The MDS Coordinator will ensure accurate RAI assessments are submitted to CMS per the RAI calendar. Must have good communication and documentation skills. This position requires someone with the ability to multi-task and can work well with the clinical team to achieve desired outcomes. Previous MDS experience is required.
