Under the direction of the Nurse Manager/Program Director, the Wound Care Coordinator communicates and collaborates closely with physicians, other WCC staff, and other members of the health care team, to provide consistent quality care for patients with wounds. Working with a high degree of autonomy, the Coordinator accepts responsibility and accountability for assigned patients as a case manager to plan, implement, and evaluate the care of the patients.
Education: Requires Bachelors degree with a major in nursing (BSN). Graduate of accredited school of Nursing. Wound Care Certification preferred.
Experience: Three (3) years of experience as staff nurse in a medical/surgical or home care setting preferred. Wound Care experience preferred.
License: Current NYS RN license.
