Short Description
This position provides, care coordination and treatment adherence services for persons at high risk for HIV infection. This provides emergent non-medical case management services for EIP patients as needed. This position has a strong emphasis on outreach to our patients
Assess all EIP clients requesting PrEP services and identify immediate/urgent needs and take appropriate action
Complete PrEP assessments as applicable
Develop client service care plans
Complete data entry for HIV testing and PrEP database
Develop client service care plans
Monitor HIV testing and Outreach Supplies
Perform HIV Counseling and Testing
Experience Required
0-2 years experience required
Communication – Ability to communicate with patients, visitors and coworkers required
Documentation – knowledgeable of computer software applications, use and data entry required
Customer service – ability to respond effectively to patient questions and concerns. Knowing when it is appropriate to escalate to supervisor required
Education Requirements
HS/GED required. Bachelor's preferred
License/Certification Requirements
Preferred:
Successful completion of HIV Counseling and Testing Training ( Will Provide Training )
Successful completion of HIV 101 basics training ( Will Provide Training )
Successful completion of PrEP Counselor Training ( Will Provide Training )
