The Criminal Justice Liaison serves as the primary bridge between behavioral health services and the criminal justice system. The liaison will work to provide release planning for inmates who suffer from mental health illness and/or substance abuse disorders. The liaison will insure clients make connections to behavioral, medical health and/or substance abuse treatment. The liaison will facilitate direct referrals for housing, employment, legal, educational, peer-support, entitlements, and all other essential community-based needs.
This is a community based position to cover Bedford, Franklin, Lincoln, and Giles counties. Mileage is reimbursed. Primary setting will be within the jails of these counties, along with office and community environments.
Essential Duties and Responsibilities:
Must value cultural differences, be able to work as a team member, and be committed to helping incarcerated individuals.
Provide assessment, advocacy and resource linkage to persons with mental illness who are incarcerated or at risk of incarceration.
Arrange for outpatient follow up services including appointments with outpatient, MD, therapy, A&D, and/or case management as needed for inmates being released from jail.
Conduct follow up activities for mentally ill inmates released from jail into community programs and services to insure successful integration into the community.
Offer Mental Health Crisis Management Training to sheriff personnel, alternative transporting agencies, other criminal justice personnel in the judicial districts, and mental health center staff.
Provide basic mental health training for the Tennessee Correctional Institute’s Basic training as scheduled by TCI in the judicial district.
Identify, screen and assess mentally ill inmates and facilitate continuity of care while the consumers are being held in jail or are in the community and at risk for incarceration.
Conduct thorough assessments of needs using state mandated assessment tools.
Prepare and maintain confidential case records and documentation in the electronic health record and Tn-Wits documentation program to ensure confidentiality and control access to sensitive information.
Submit monthly statistical reports in a timely manner.
Consult with and attend meetings with service providers and community partners on resource-related issues.
Attend and participate in TDMHDD quarterly network meetings.
Collaborate with clinical team to perform risk management and crisis intervention services as necessary.
Develop network of trusted contacts of organizations and agencies in which to refer and coordinate care.
Participate in multidisciplinary team meetings.
Bachelor’s Degree in Social Work, Sociology, Psychology or related field required; Master’s Degree preferred.
1+ year’s case management experience; 3+ years preferred. 1+ years in a criminal justice affiliated environment.
Knowledge of and experience working with local social service, public health, and criminal justice system.
Experience working with high risk/high need population; preference for crisis and legal experience.
Must demonstrate leadership capacity, ability to work within a team, but also be able to work independently.
Excellent written and oral communication skills.
Must be highly organized, detail oriented, and reliable.
History of documenting within an electronic health record preferred.
Knowledge, Skills, and Abilities:
Must be able to remain calm in the midst of difficult circumstances and emergencies. Flexible and able to adapt to change. Ability to respond in a professional manner in all situations.
Ability to work with all clients regardless of past criminal charges or convictions.
Ability to communicate clearly and effectively via oral or written means.
Ability to present a friendly and positive demeanor to all clients, their families, staff, court/probation/jail personnel, treatment providers, and the general public.
Ability to make oral presentations and use PowerPoint to train law enforcement.
Working knowledge of Outlook, Word, Excel, and PowerPoint. Ability to think analytically and evaluate the impact of case management recommendations.
Ability to set work priorities and to evaluate and create solutions to work related problems.
Strong math skills. Ability to assist clients in developing a budget.
Crisis management skills preferred.
Knowledge of case management services and local community resources.
Other Duties and Responsibilities:
All other duties as assigned. Must be flexible and able to adapt to a changing work environment.
