Tri Valley Care is seeking a Community Specialist to join our Intellectual and Developmental Disabilities Services (IDD) division in Carbondale, PA.


Responsible for helping individuals with intellectual/developmental disabilities and/or autism access supports and services within their community. The main function of this position is to assist and support individuals in maximizing their "Everyday Life." The principles and practices of Positive Approaches and Self-Determination will be the driving influences for the job and work will be guided by "Charting the Lifecourse" tools.

This position will have the responsibility of utilizing tools from LifeCourse framework to help the individual (and their family or support network) map their version of a meaningful life and goals; This information will then drive the development/updates of their Individual Support Plan (ISP).

This position will maintain regular communication and contact with each individual and their support network in order to ensure their wants are heard and needs are met. The individual [or appropriate designee] will drive the focus and direction of the planning process.

Will need to become highly versed in the LifeCourse Framework and the use of Charting the Life Course tools. They need to be knowledgeable of current best treatment and supportive case management practices and also knowledgeable of community resources within and around Lackawanna and Susquehanna Counties.

Fee for Service position with pay rate $15/hr non-billable and $18/hr billable

  • Bachelor's Degree from an accredited college or university and at least 4 years' experience in public or private social work; Course work must include the completion of at least 12 semester hours of college-level courses in sociology, social welfare/social work, psychology, gerontology or another related social/behavioral science.
  • Able to demonstrate prior knowledge, experience and competency working with individuals who have an intellectual/developmental disability
  • Valid driver's license
  • Must be cleared through Criminal, FBI and child abuse clearances
  • Flexibility of time and work hours is necessary in this position. There may be a need for evening and/or weekend hours dependent on the needs of the individuals receiving support.
