We are seeking a Hospice CNA to join our team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients.

Hospice Philosophy:
Hospice care is an interdisciplinary approach designed to meet the needs physical, psychological and spiritual of a person at the end of life with the goal of comfort and not cure. Hospice considers the unit of care to extend beyond the patient and include the family and caregivers as defined by the patient.
Responsibilities:
On an average day as a CNA, with Heart 2 Heart Hospice Care, you will work with our clinical team to provide patient care. Hospice care provides patients with the comfort and dignity they deserve through their end-of-life journey. This role requires daily driving in order to provide care where our patients call home, this could be a home setting, facility setting, etc.
Administer nursing care to ill, injured, or disabled patients
Diagnose and establish patient treatment plans
Monitor and report changes in patient symptoms or behavior
Communicate with collaborating physicians or specialists regarding patient care
Educate patients about health maintenance and disease prevention
Facilitate referrals to other healthcare professionals and medical facilities
Maintain accurate patient medical records
Provide advice and emotional support to patients and their family members
Daily Tasks:
Spend one-on-one time assisting hospice patients with daily living activities (Personal hygiene, bathroom needs, physical activity, etc)
Report changes in the patient's conditions or environment to the attending nurse/care team
Support patient teaching based on patient care plan supported by RN, Social Worker, and Chaplain
Prepare and submit documentation of observations, findings, and services provided
Provide social interaction and reassurance to the patient and family
Perform additional, related duties as needed
Qualifications:
Valid CNA certification required
Valid driver's license, auto insurance, and transportation in order to travel to the patient's location
Verbal and written communication skills
Ability to complete patient charting on a daily basis
Job Types: Full-time, Part-time
Pay: $23.00 - $25.00 per hour
