InTouch Home Care Inc., is a Home Health Agency located in Santa Clara. We serve patients in 9 counties in and around the Bay Area. We are looking for a dedicated professional who is proficient at multi-tasking and completing tasks in a timely manner.
The Administrative Assistant, Intake/ Care Coordinator is responsible for managing all aspects of the patient intake process including assignment and management of each patient’s care team, establishing and maintaining positive relationships with patients, clinicians and referral sources, responding to patient requests and concerns, and managing insurance verification and authorization processes. They are also responsible for ensuring that clinical visits are completed in a timely manner, and consulting with the Director of Clinical Care to ensure care coordination and documentation are being completed in compliance with CMS regulation.
