InTouch Home Care Inc., is a Home Health Agency located in Santa Clara. We serve patients in 9 counties in and around the Bay Area. We are looking for a dedicated professional who is proficient at multi-tasking and completing tasks in a timely manner.

Summary:

The Administrative Assistant, Intake/ Care Coordinator is responsible for managing all aspects of the patient intake process including assignment and management of each patient’s care team, establishing and maintaining positive relationships with patients, clinicians and referral sources, responding to patient requests and concerns, and managing insurance verification and authorization processes. They are also responsible for ensuring that clinical visits are completed in a timely manner, and consulting with the Director of Clinical Care to ensure care coordination and documentation are being completed in compliance with CMS regulation.

Essential Job Functions/Responsibilities Include, but are not limited to:

  • Completes all daily patient referral and intake operations including:
  • Verifying patient information
  • Verifying insurance information
  • Assigning clinicians to the case
  • Directing the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy
  • Coordinates care between clinicians, patients, and physicians:
  • Faxing and tracking physician orders
  • Filing paperwork and uploading information into the EMR system
  • Addressing patient concerns, comments, or complaints
  • Addressing concerns of clinicians
  • Maintaining good working relationships with physician offices, hospital case managers and other referring sources
  • Following up with patients to ensure that their needs are being met
  • Maintaining good working relationships with field staff

Qualifications:

  • Experience in home health care preferred
  • Good communication, negotiation, and public relations skills
  • Autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities
  • Medical background

Bilingual, Spanish preferred

Job Type: Full-time

Salary: $11.00 to $25.00 /hour
