POSITION SUMMARY:
Reporting to the Director of Home Services (DHS), also operating as the Director of Patient Care Services, the IDT Team Manager is responsible for the effective and efficient daily supervision of their clinical team. by overseeing all patient care services, ensuring continuity and quality of care for both facility and home patients. The IDT Team Manager has the responsibility to motivate and inspire staff, foster a positive work environment.
Administrative
- Evaluates the staffing requirements and recommends changes in the staffing patterns to DHS.
- Prepares the monthly schedule in coordination with the scheduler and other Team Managers as appropriate.
- Reviews productivity report, position control and SHP data on a regular basis to ensure agency goals are being met.
- Upholds and enforces company policies and brings questions and recommendations for change to senior leadership.
- Reviews and approves timecards, and pays particular attention to overtime, and missed meals. Utilizes the EMR and HRIS systems in combination to regularly track and address timely inconsistencies.
- Reviews and approves the time off requests, ensuring adequate patient care coverage in coordination with the scheduler and other team managers as appropriate.
- Reviews and ensures that assignments and scheduling of all team members effectively meet the needs of their assigned patients, while remaining compliant with organization policies and procedures..
- Recruits, screens and interviews candidates for open positions and participates in orientation of new personnel via documented mentoring and precepting models within the team.
- Coordinates with the intake/community liaison nurse the assignment of the case managers available for new admissions. Participates in the establishment, revision and implementation of administrative policies and procedures
- Participates in collective bargaining as required, working with CHRO to address grievances.
- Ensures accountability of direct reports through regular tracking and documented coaching, training and counseling.
- Facilitates or delegates responsibility for interdisciplinary team meetings (IDT) that effectively meet regulatory and organization standards.
- Prepares the agenda for monthly team meetings.
- Coordinates home health aide oversight by case managers.
- Understands the contracts with providers for delivery of services, such as pharmacy, durable medical equipment, and medical supplies, and monitors pharmacy and DME expenses. All outliers identified will be brought to the attention of the DHS along with an action plan for a solution to remedy the situation.
- Ensures compliance with Federal and State regulations and accreditation standards.
- Monitors documentation and ensures compliance with state and federal regulations, accreditation standards, corrective action plans and agency policies and procedures.
- Participates in education support relevant to EMR revision and monitors staff for compliance with any changes.
- Participates in the development of the Clinical Services annual goals and objectives, including quarterly evaluations, offering support, identifying strategies and monitoring of staff for performance toward goals and objectives.
- Participates in the ongoing evaluation of the clinical component of the computerized patient care system and brings attention to perceived barriers interfering with efficient and timely documentation.
- Ensures staff members document in the home/facility and if unable, ensures that documentation is completed per policy.
- Performs service recovery and completes complaint investigations within organization’s stated timelines.
- Actively participates in Performance Improvement Projects.
- Collaborates with Q&E to support the QAPI program by reporting out on audits relevant to the home program.
- Assists the DHS with budgeting and financial planning.
- Participates in Administrator On-Call (AOC) rotation.
Clinical
- Performs field observations approximately 3 times per year of each team member.
- Provides clinical coaching to staff through joint field visits.
- Identifies educational needs of staff and establishes opportunities to address needs, including referrals to the Education team.
- Holds staff accountable to performance markers and provides performance coaching to staff, as needed, up to and including disciplinary action.
- Acts as resource to the staff regarding clinical issues that arise.
- Participates in the quality review process regarding utilization management, including review of new patient documentation within 3 days following admission & recertification review of documentation for ongoing patients.
- Ensures the quality of patient care and patient safety practices.
- May provide direct care of patients as required during times of critical staffing shortage, including performing Admissions, as needed.

- Knowledge of private insurance, Medicare and Medi-Cal regulations.
- Knowledge of State and Federal regulations and accreditation standards.
- Must be able to work well under pressure meeting multiple priorities and sometimes competing deadlines.
- Demonstrates cooperative behavior with colleagues and management staff.
Experience and Education
- Current California registered nurse license.
- Certified Hospice & Palliative Care clinician preferred.
- Experience working as an RN in a Skilled Nursing Facility preferred.
- MSN/BSN preferred; Associate’s degree with 2-3 years of direct supervisory experience in home health or hospice is acceptable alternate qualification.
- At least 2 years’ experience working as registered nurse caring for the terminally ill.
- Prior management experience desired, preferably in a hospice/home health care agency.
- Knowledge and understanding of Medicare, Medi-Cal and private insurance hospice benefits.
- CPR certification required.
- Current California driver’s license and car in good working condition required.
