  • Meets with residents and families to develop care plans
  • Coordinates work assignments for resident care partners
  • Coordinates care staff schedules ensuring adequate coverage
  • Covers Caregiver and/or Medication Assistant shifts occasionally (as needed)

  • Possess the ability to handle complex issues
  • Demonstrate strong interpersonal communication skills
  • Additional duties as assigned

Required Qualifications

  • Ability to demonstrate excellent communication, approaches, and behavioral support for residents living with dementia
  • Ability and willingness to coach/train caregivers to care for residents living with dementia
  • Licensed Practical nurse in the state of Arizona
  • Minimum of one year experience in long term care, community based or assisted living
  • Minimum of one year experience in a supervisory role
  • Staffing experience preferred
  • CPR Certification
  • Must work a full shift, come to work on time, and work overtime as needed
  • May be required to work evening/night shifts from time to time and/or stop in during night shifts to meet with staff and provide supervision, leadership and support
  • The ability to work according to the necessary schedule to meet job requirements with or without reasonable accommodation is an essential function of this position

Salary: $25.00 to $28.00 /hour


  • Assisted Living or Skilled Nursing: 2 years (Required)
  • Supervisory/Leadership: 1 year (Required)
  • Care Plan Development: 1 year (Preferred)
  • Staffing/Scheduling: 1 year (Preferred)
