Gallagher Home Health is hiring a Weekend Only Home Health Physical Therapist. This will be an Admission Therapist/Start of Care position to support our current staff in the South Hills (Bethel Park, South Park, Bridgeville, Mount Lebanon areas). This is a flexible weekend position looking for Friday, Saturday and Sunday or Saturday and Sunday or Saturday, Sunday and Monday.
Job Description
The Physical Therapist (PT) is responsible for the evaluation of the needs of the patient, planning and coordinating the patient’s care, and developing and following the physician’s Plan of Care. The PT provides services to the home health patient with neurological, musculoskeletal, orthopedic, and cardio-pulmonary impairments, fine motor, sensory motor, and developmental delays, traumatic injuries and swallowing disorders. The PT is responsible for directing, supervising, evaluating and providing physical therapy services to patients in their homes as prescribed by the attending physician.
Ability to:
- Demonstrate excellent customer service, clinical judgement, critical thinking, problem solving, organization and planning skills
- Follow County, State and Federal regulations
- Establish and maintain effective working relationships with employees, patients and families, other agencies and the public
- Communicate effectively verbally and in writing
- Maintain and promote a positive attitude
- Maintains and completes all documentation promptly and accurately including visit assessment, OASIS, 485 and Verbal Orders on the laptop device or in written documentation while in the patient's home and within the agency and regulatory standards.
QUALIFICATIONS FOR THE PHYSICAL THERAPIST PT POSITION:
- Graduate of a state approved school of physical therapy accredited by the American Physical Therapy Association
- Licensed as a Physical Therapist in the Commonwealth of Pennsylvania
- Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive vehicle with insurance limits as established in personnel policies
