The Patient Care Coordination Center at the Alvin J. Siteman Cancer Center at Barnes-Jewish Hospital and Washington University School of Medicine provides access for physicians and patient's to make referrals to any of the specialist providing care at the Siteman Cancer Center. The Patient Care Coordination Center staff provide assistance to internal and external physicians requesting consultations for patient's requiring appointments with Medical Oncology, Radiation Oncology and Surgical Oncologist. Through the efforts of the Siteman Cancer Center Outreach and Marketing team, the Patient Care Coordination Center is represented as a ""one-stop"" provider for consults requested by a referring physician to the Cancer Center. Multidisciplinary appointments are scheduled to meet the needs of the patient and the referring physician. Navigation assistance is provided for newly diagnosed cancer patients coming to the cancer Center for the first time.

Role Purpose

Coordinates the health care of select patient populations/designated service across the continuum of care. Responsible for coordination of patient contact, patient education, prepares patient/family for procedure/test in collaboration with the healthcare team.


Facilitates patient care by assessing and evaluating patient's physical, psychosocial and emotional needs.
Coordinates the delivery of safe and appropriate patient care governed by evidence-based clinical practice.
Monitors care and data to optimize patient outcomes.
Facilitate the education of patients and families. Acts as a resource to the health care team.
Ensures actions meet and support the overall goals of the business.
Minimum Requirements


Nursing Diploma/Associate's
