POSITION TITLE: Social Worker
REPORTS TO: Patient Care Manager
FLSA: Exempt
Position Scope:
The Social Worker, under the direction of the Patient Care Manager and clinical supervision of the Access & VH Cares Supervisor, provides guidance, support and counseling to patients and families dealing with the psychosocial or socioeconomic issues involved with the terminal illness in the home, assisted living, Care Centers and nursing facility settings.

I. Essential Functions/Performance Factors
· Assesses emotional factors related to terminal illness.
o Complete the initial psychosocial assessment within agency time frames.
o Develop and implement a plan of care that is related to identified patient/family needs.
o Continually evaluate the plan of care for effectiveness of interventions and for the purpose of making appropriate changes.
· Assists the physician and other IDT/IDG members in recognizing and understanding the social/mental stress and/or disorder that exacerbates the symptoms related to terminal illness.
o Promote patient self-determination.
o Assist with completion of Advance Directives as instructed.
o Provide alternatives to unsafe environments.
o Assist patient/family in ensuring continuity of patient care when patient is transferred from one care setting to another.
o Assist patient/family with ensuring continuity of patient care for patient’s without primary caregivers.
· Advocates for patient/family.
· Counsels the patient/family regarding effects of serious illness, death & grief.
o Follow identified interventions in the care planning process.
o Utilize agency resource related to coping and grief.
o Evaluate needs of any children in family related to grief and loss issues.
· Acts as a liaison to community resource network.
o Utilize the Community Resource Book.
o Interact on community level through public speaking and networking.
o Facilitate nursing home transfers.
· Prepare clinical and progress notes in an accurate, complete and timely manner.
o Complete documentation of each social work visit within 24 hours of visit.
o Complete the social work assessment record accurately.
o Write social work notes that reflect the patients terminal illness according NHPCO guidelines, Local Medical Review Policies and the care plan.
o Sign all entries with full signature & credentials.
o Complete the documentation within agency time frame and reflect an overview of the care provided.

Education and Work Experience:
- A Social Work Degree from a school accredited by the Council of Social Work. BSW Required.
- One to two years of related experience
- Evidence of clinical experience and skills necessary to perform job
- Exhibits excellent communication skills
Certifications and Licenses:
BSW, WV and OH
Certified Hospice & Palliative Care Social Worker (ACHP-SW)) after two years of hospice experience
Current driver’s license, automobile insurance, and reliable transportation

WORK ENVIRONMENT
While performing the duties of this job, the employee frequently is exposed to outside weather conditions when traveling between sites. Patients’ home environments are variable with regard to heat, noise, air circulation and conditioning, space, cleanliness, accessibility and all other environmental factors. Inpatient and skilled nursing facility environments are standardized. Office environment in a controlled atmosphere building.

Pay: $50,000.00 - $58,000.00 per year
