Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a part-time (20 hrs/wk) Peer Specialist/Case Manager in Worcester, MA.

The Program for Assertive Community Treatment, or PACT, is an intensive and highly integrated evidence based practice for community mental health services. Our team serves 80 individuals age 18 + who struggle with serious and persistent mental illness and whose symptoms result in serious functioning difficulties in several major areas of life, often including work, social relationships, residential independence, money management, and physical health and wellness.

Unlike other community-based programs, PACT is not a linkage case-management program that connects individual’s services. Rather, it provides highly individualized services directly to clients. PACT is multi-disciplinary team model who whose goals are to support clients to reduce hospitalizations and to restore and enhance the emotional well-being and level of social functioning of clients. Services are provided in clients’ own homes and communities and are provided 24 hours a day, seven days a week, and 365 days a year. This program serves clients in Worcester and its surrounding areas.

Responsibilities
Provide peer counseling and support, drawing on common experiences as a peer to validate persons' served experiences, provide guidance and encouragement in his/her recovery path.
Serve as a mentor to persons served to promote hope and empowerment.
Acts as an interpreter to help non-mental health team members better understand and emphasize with each persons' served unique and subjective experience and perceptions.
Provide expertise and consultation from a mental health consumer perspective to the entire team concerning persons' served experiences with symptoms of mental illness, the effects and side effects of medications, opinions and responses to treatment and experiences of recovery.
Collaborate with team members to promote a culture of self determination.
Help persons' served and team members identify, understand and combat stigma.
Increase awareness of and support persons' served participation in self help and advocacy organizations that promote recovery.
Serve as a liaison between the team and RLC, self help, etc.
Assists in the direct provision of services to persons' served by the team including side by side support for accomplishing identified objectives, skill teaching, assistance with activities of daily living, coping skills, etc.
Completes documentation in a timely manner.
Attends scheduled meetings and trainings
Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.

Qualifications
Grade 5: Case Manager I: HSD/HiSet/GED required; Associates Degree preferred
Grade 6: Case Manager II:Requires a Bachelor’s degree; preferably in the field of psychology or human services
1-2 years experience preferred
Previous participation in Mental Health system as recipient and willingness to convey this experience as relevant is required
Must be certified or be eligible for the Mass Peer Specialist training and be certified within 6 months of being hired
Current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in your own vehicle or a company vehicle
Ability to demonstrate clear boundaries with clients while being client centered and supportive.
