As a Licensed Practical Nurse with our agency, we are seeking someone to join our team to help facilitate nursing functions that supports individuals with intellectual and developmental disabilities and their well-being. In this position, you will be working in a team environment within the Nursing Department. This position as the LPN will facilitate physical assessment functions, document assessments appropriate to licensure, care planning, and medication administration monitoring. The LPN will be responsible for providing employee training on medical support for clients and to employees on medical and/or health objectives specific to the needs of the client’s support. In this position, the medical environment will be conducted in an in-home care environment.
Essential Functions:
- Perform and respond to physical assessment functions for caseload.
- Provide support and training to employees to ensure people supported are safe, medical concerns are addressed and person supported experiences services of continuity.
- Monitor the medication administration process to ensure individual supported has the best possible health, including: medication orders and consulting with pharmacist on Pharmacy Consultation contract, review and respond to med errors timely and provide re training for employee error, MAR’s and Count Sheet monthly reviews, monitor and assure med passing, and conduct random monitoring or med administration.
- Ensure medical and health needs are met to ensure the best possible health, including: responding and monitor medical concerns timely, perform tasks outlined in medical procedures and protocols, scheduling medical appointments and arrange transportation, assistance with hospitalizations, after care, and on-going monitoring upon discharge, review medical documents.
- Provide support and monitoring of individual supported to be involved with their medical care and be communicated at their developmental level for caseload.
- Oversee access to and maintenance of medical equipment to ensure people supported on caseload have access to their environment, and necessary assessment/interventions to be completed.
- Communicated information regarding individual served following confidentiality and HIPAAA privacy and security policies for caseload.
- Other medical objective is subject to be assigned need to ensure safety and wellbeing of individual supported.
Qualifications:
- Graduate degree in Practical Nursing.
- Current and in good standing LPN Licensing in the State of North Dakota
- Current and valid driver's license and proof of auto insurance.
- Successful completion of criminal background, child abuse and neglect check(s), credit check, and pre-employment drug testing.
- Required to completed Developmental Disabilities Certification from Minot State University within six months of hire date and Van Driving Certification within one month of hire date.
- Excellent verbal and written communication skills.
- Experience with ID Teams, T-logs, and MAR’s is preferred.
- Clinical experience along with experience with working with individuals with intellectual and developmental disabilities is preferred.

Pay: From $24.06 per hour
Expected hours: 40 – 45 per week
