Key Responsibilities:
- Assess and treat a variety of wounds, including pressure ulcers, diabetic ulcers, surgical wounds, and traumatic wounds.
- Develop and implement individualized care plans for wound management.
- Collaborate with healthcare professionals, including physicians, nurses, and therapists, to ensure comprehensive care.
- Educate patients, families, and staff on wound prevention and care practices.
- Maintain accurate and detailed documentation of wound assessments, treatments, and outcomes.
- Monitor and evaluate the effectiveness of wound care interventions and adjust plans as necessary.
- Stay current with best practices and advancements in wound care.
Qualifications:
- Valid RN or LPN license in the state of Illinois.
- Certified Wound Care Nurse (CWCN) or equivalent certification preferred.
- Minimum of 2 years of experience in wound care, preferably in a skilled nursing facility.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team-oriented environment.
