24/7 AvaRe Healthcare is seeking for an ambitious Registered Nurse who is looking for growth within a company, managing home health care nurses.
We are conveniently located in an A-Class Building in the desirable north end of the Denver Technological Center near the intersection of I-25 and I-225 with convenient access to Interstate 25 and immediate access to Interstate 225. Direct access to DTC Transfer Station for bus lines and nearby Light Rail stations. The building offers an executive health club with showers and lockers and an onsite cafeteria.
As a Nursing Supervisor, you assist the Director of Clinical Services with the supervision of home care services to patients in their place of residence and strive to provide the highest quality of care.
A Nursing Supervisor is available at all times during operating hours and participates in all activities relevant to the professional services furnished, including the development of qualifications and the assignment of personnel.
Nursing Supervisor Qualifications:
- Two (2) years home health nursing experience, minimum,
- At least three (3) years healthcare experience in the field of supervision in home care or a closely related service,
- A current license as a Registered Nurse, RN in Colorado,
- Knowledge of home health rules and regulations,
- Computer skills.
- Manages a group of clinicians on a daily basis and oversees their case management of patient load and patient documentation,
- Reviews initial referrals and assists with obtaining starts of care and roc orders,
- Supports clinicians in communication with MD and following all expectations according to state and federal rules and regulations and agency processes,
- Performs training for clinicians and ensures team of clinicians are up to date on training and education,
- Performs monthly and as needed case conferences with team of clinicians on their patient case loads,
- Processes all documentation, plans of care and orders daily,
- Assists with QA program, such as meetings and chart audits, as needed,
- Assists with on call administrative duties and is available for clinical questions and review of new referrals on scheduled on call evenings and weekends.
