A supervisory position providing support to the Resident Care Coordinator/Administrator/Director as it relates to organizing medical charts, filing, scheduling doctor’s appointments, monitoring podiatry, dental and other ancillary services, appropriating necessary forms for nursing stations, distribution of supplies, transportation to and from doctor’s visits (upon insurance approval) and orientation of new employees and residents.


  • Education: High School graduate or G.E.D. equivalent;
  • Successfully complete medication pass training class;
  • Skills:
  • Ability to communicate with residents and others in the English language
  • Ability to read and write English at a level sufficient to
  • Prepare staff schedules and daily assignment sheets
  • Monitor resident records and maintain proper documentation
  • Update policies as needed to improve resident services (with Administration approval)
  • Ability to lift up to 50 lbs without mechanical assistance. Ability to bodily support with minimal assistance (i.e. partial weight bearing, etc) up to 100 lbs.
  • Ability to visually observe residents (pupils dilated, etc.)
  • Ability to audibly hear call bells, emergency systems and verbal needs specified by residents
  • Ability to turn, stoop, bend, stretch in order to assist residents
  • Ability to push wheelchairs and med carts down halls without assistance
  • Ability to stand and walk for prolonged periods;
  • Ability to manipulate equipment (blood pressure, temperature, accucheck, etc.)
  • Ability to recognize and react to emergencies (i.e. Evacuation of residents, guests and co-workers in even to fire, etc.)
  • Ability to legally drive facility vehicle upon insurance approval
  • Personal Qualifications:
  • Must be 21 years of age or older
  • Must be good moral character as evidenced by three current letters of reference
  • No known communicable disease and annual negative TB testing
  • Must provide current criminal record


  • Monitor resident records daily including MAR’s and all documentation on assigned hall;
  • Skills Performance Checklist;
  • Monitor Personal Care Service logs daily;
  • Monitor Facility;
  • Assist with Agencies, Doctors, Nurses, Etc.;
  • Post newly prescribed medications, note discontinued medications (posted/dated, initialed) on MAR’s daily;
  • Pull Discontinued medications from carts, verify notation on MAR’s daily;
  • Monitor medical charts daily;
  • Take care of Emergencies;
  • Keep diets posted at Nurse’s Station and in Kitchen making dietary department aware of any changes;
  • Monitor food service during meals;
  • Assist with medication pass when necessary, monitor med-tech during med passes periodically;
  • Monitor room laundry;
  • Meet with Administrator/Director daily to discuss any changes in level of care;
  • Assist RN with assessments, Etc.;
  • Complete Care Plans;
  • Keep FL-2’s up to date;
  • Work with Enhanced Care Workers (DSS) and follow-up on all Enhanced residents; and
  • Inform home office of any ADL changes through administrator or office manager.
  • Allow residents freedom of movement;
  • Provide residents assistance with bathing, dressing, eating, etc. as needed;
  • Provide residents assistance with personal care needs in grooming such as hair, nail, teeth, etc., as needed;
  • Assure residents are dressed appropriately when in living areas of facility or outside the building
  • Assist and teach residents skills in self-help (Ex. feeding, bathing);
  • Assist residents into and out of bed, wheelchair as needed;
  • Assist residents to dining room, bathroom, activities room, outside building, etc. as needed;
  • Provide personal attention such as bedside water and ice or other personal attention as requested or required;
  • Respond to hand bells or other signaling devices;
  • Assist Activities Coordinator in carrying out planned activities;
  • Assist with transportation when so directed by Administrator (proper driving authorization required prior to driving);
  • Provide residents assistance to keep clothes in proper order;
  • Assist residents with a clean towel and wash cloth as needed;
  • Provide assistance with using and emptying bedpans, urinals and bedside commodes as needed;
  • Report all incidents/accidents to SIC, PCC or Administrator/Director immediately; and
  • Assure residents are smoking in outside designated areas only.

  • Medication Administration in Assisted Living: 3 years (Preferred)
