General Description: In the home of the Sisters of Mercy the Charge Nurse is responsible for implementing the plan of care to meet the assessed needs of each resident providing the delivery of quality clinical services in an atmosphere of dignity and compassion. The Charge Nurse performs assigned nursing procedures for the comfort and wellbeing of all residents and provides a variety of patient care functions within the scope and practice of her/his nursing license. The Charge Nurse on duty is be responsible for ensuring that all patient care duties are carried out by the CNAs, NAs and HHAs on duty. Principal Duties and Responsibilities:
· Ensures person-centered care, putting the resident, and their loved ones at the heart of all decisions, focused on wellness and comfort.
· Ensures a home environment is maintained focused on wellness and comfort, and adapted it to meet the varied needs of each resident.
· Assists in updating and ensures the implementation resident’s plan of care.
· Checks, evaluates, monitors, and records vital signs, intake and output, elimination pattern, weight, condition changes, and patient reactions.
· Administers medications and treatments in accordance with the accepted standards of professional nursing practices and the departmental policies and procedures.
· Provides all care in a respectful way with an alertness to resident’s changing needs.
· Ensures the residents’ right to privacy and to be treated with dignity.
· Maintains resident confidentiality.
· Provides the residents assistance in dressing, undressing, bathing, eating, grooming, repositioning, ambulation and all other tasks related to safety, comfort and well- being as able.
· Assures proper nutrition and hydration for the resident’s.
· Assesses resident’s condition and takes appropriate action.
· Reports any changes in the resident’s condition to the Health Services Coordinator and the physician or PACE program nurse.
· Facilitates arrangements for transfer of a resident to the hospital emergency room following department policy and procedure.
· Prepares resident’s forms for doctor’s visits.
· Takes and transcribes physician orders.
· Ensures the scheduling and follow up of resident appointments: personal, medical, therapies, diagnostic procedures, lab work, and the like.
· Accompanies primary provider and other service providers on visits to residents.
· Documents care according to the professional nursing practices and department policies.
· Provides shift report to CNAs, NAs, HHAs, and updates them on changes in resident care throughout the shift.
· Delegates and supervises resident care by the CNAs, NAs, HHAs, ensuring thorough, sensitive, compassionate, and understanding caregiving is provided and completed with in a timely and efficient manner.
· Educates staff in the provision of safe, thorough, and sensitive caregiving for the resident with dementia; utilizing the knowledge and tools gained through the Train the Trainer program.
· Ensures staff participates with and assists residents during the Activities and Enrichment programming provided.
· Responsible for any disciplinary action(s) necessary, including sending the employee out of the building if needed, with immediate notification of the Health Services Coordinator or her designee.
· Orients and trains new staff to the charism and vision of the Sisters of Mercy and their job description.
· Functions as the first tier of supervision of CNAs, NAs, HHAs and reports issues and concerns to the Health Services Coordinator or her designee.
· Assists in replacement of staff call-offs to ensure adequate staff to meet resident needs.
· Participates in continuing education opportunities and in-service programs.
· Uses proper body mechanics in performance of duties.
· Demonstrates appropriate use of equipment necessary for performing her/his duties.
· Demonstrates and ensures the appropriate use of two person lifts for resident transfers.
· Assures safety of residents from elopement checking regularly and responding immediately to safety alarms.
· Demonstrates appropriate use of fire and safety plans, and use of safety devices.
· Follows universal precautions and endures appropriate precautions are followed by staff and visitors.
· Reports to the on-coming shift any changes in a resident’s condition and the nursing and medical measures implemented to meet the resident’s needs.
· Ensures all narcotics are counted with the oncoming charge nurse to ensure that the narcotic count is accurate. Discrepancies are reported to the Health Services Coordinator or her designee before handing over the narcotic keys to the oncoming nurse.
· Updates the Health Services Coordinator on a regular basis in regard to any resident or personnel concerns, needed equipment and supplies, or other matters of importance.
· Maintains the order, adequate supply and cleanliness of areas utilized by staff, including but not limited to, office, supply, linen areas; medication cart and containers; staff lounges/break rooms.
· Provides additional care and services as requested by the Health Services Coordinator/Clinical Nurse Manager/Nurse Manager.
· Performs other duties as necessary.
Education & Experience:
· Experienced Licensed Professional Nurse or Registered Nurse with a valid license with in the state of employment.
· Experience in the field of nursing and/or gerontology.
· Experience working with persons experiencing dementia.
· Experience in supervising, and delegating.
· Experience in a position where nursing assessment skills were needed.
· Computer skills: knowledge in Microsoft Word, and Excel, Email capability.
Qualifications, Knowledge and Skills Required:
· Current basic life support training.
· Preferably prior experience in a healthcare organization.
· Ability to cultivate effective working relationships with a broad range of employees and external contacts.
· Ability to work independently and also function in a team environment.
· Ability to supervise, delegate and communicate.
· Strong organizational skills and the ability to establish priorities.

Americans with Disabilities Act (ADA) Requirements:
· Physical: Able to push, pull, lift or carry objects up to 50 pounds. Perform moderately difficult motor skills such as operating computer keyboard. Frequently walk, stand and sit for prolonged periods of time. Ability to file documents.
· Sensory: Able to read fine print in written documents and use visual discrimination in reading documents, including color discrimination. Distinguish normal sounds with some background noise, as in answering telephones, pagers, etc. Able to speak clearly and make self understood while also understanding others using the English language.
· Reasoning/Cognitive Ability: Frequent need to understand and communicate concepts behind specific ideas and the theories behind several related concepts. Must be able to frequently remember multiple tasks/assignments given to self and others over long periods of time with frequent interruptions. Must be able to continue work started by another individual.
· Environmental Conditions: Functions in a general office environment under normal conditions. Frequently exposed to normal range of temperatures. Minimal exposure to hazardous chemicals. Frequent exposure to electro-magnetic radiation as in CRT’s.
· Equipment: Ability to operate personal computers and standard office/medical equipment.
· Mathematical Skills: Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, and percentages. Ability to apply basic math concepts.
· Language Skills: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, procedures, manuals, and financial reports. Ability to write routine reports and correspondence. Ability to speak effectively with other employees, members, and supervisors.
