SUMMARY: To assist in the coordination of personal care services for a smooth transition and improved quality of life for the patient in the patient’s home. To ensure quality and safe delivery of home health care services in accordance with Agency philosophy and established standards of home health care and nursing practice.
Duties and Responsibilities:
Coordinating care services for patients in the communities and private homes.
Communicates with social worker, clients and families to deliver services based on their approved care plans.
Contacts patient/patient’s family to introduce services of TMT and answers questions.
Maintains confidentiality of patient's information.
Maintain relationships with referral sources and communities.
Continue building relationships with congregate sites, hospitals and extended care facilities as needed.
Performs the role of liaison between all systems for continuity of patient care.
Gets involved in community health events to promote healthy living for our seniors.
Gathers and shares comprehensive listings of all community services available to home care patients and their families as requested.
Helps patients and families to understand, accept, and follow care plan authorizations and provides services planned to maximize individual capabilities.
Assists in resource planning by making arrangements with other supportive services for which the patient is eligible.
Participates in discharge planning activities and prepares appropriate service delivery.
Implements appropriate infection control and safety measures and reports all hazards and accidents in a timely and accurate manner.
Maintains client charts with necessary information.
Participates in professional growth and development.
Attends Agency in-services as required.
Participates in educational programs appropriate to functions.
Participates in verifying visits from caregivers and gathers data for the billing department as needed.
Participates active in appropriate professional and community organizations to enhance knowledge of resources available to patients and their families.
Participates in Performance Improvement plans for caregivers, Annual Performance Evaluations, and recommends terminations when necessary.
Participates in recruiting, interviewing and hiring process.
Participates in ensuring that the caregivers credentials are up to date.
Fulfills additional performance responsibilities as assigned and as necessary.
Participates in On-call rotation as scheduled.
High school diploma or equivalency required.
Two (2) years of customer service experience preferred.
High School Diploma or equivalent.
Prior Care Coordination experience preferred.
Problem solving abilities while multi-tasking required.
Ability to read and write English. Basic computer skills required.
Use hands to grip or feel.
Reach with hands and arms.
Occasionally required to bend, stoop and kneel.
Frequently required to stand on one’s feet for a long period of time.
Ability to deal effectively with stress.
Works within an office environment, working with computers, computer systems and other office related equipment.
Sit, stand, bend, lift, and moves intermittently during working hours.
Communicates and interacts with supervisors, peers and subordinates under all conditions and circumstances.
Communicates with people outside the organization, representing the organization to customers, the public, government and other external sources, as required.
Works in the field visiting client’s homes as needed for supervisory visits.
