Santé Chandler is seeking a full time Occupational Therapist (OT). Shift is Monday-Friday with 1 weekend day rotation per month. The Occupational Therapist is responsible for providing and directing Occupational Therapy services to guests, including assessment, treatment, program planning and implementation, related documentation, and communication and functions under physicians’ orders.
Provides treatment in accordance with established standards of practice, and department procedures.

• Conduct screenings of guests newly admitted to the facility and referred by nursing to determine need for intervention/treatment.
• Evaluates guests to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated.
• Interprets and communicates evaluation findings to guests, family, and other team members.
• Establishes individual guest schedules in conjunction with other team members.
• Develops treatment plans by identifying a problem list, including short and long term goals and methods to achieve identified goals.
• Implements and conducts treatment as outlined in treatment plan.
• Monitors guest’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.
• Develops appropriate home or community programming to maintain and enhance the
performance of the guest in their own environment.
• Documents results of guest’s assessment, treatment, follow-up, and termination of services.
• Adapts treatment to meet the individual needs of the guest.
• Provides/recommends/fabricates adaptive devices, orthotics or prosthetics. Trains guest, family, and appropriate staff in the use or application of the above.
• Participates in discharge planning.

• Prefer three (3) years of experience. Require a minimum of one year Occupational Therapy experience in an acute care hospital, facility or rehab setting.
• Bachelor or Masters required, prefer Doctorate of Occupational Therapy from an accredited program.
• Successful completion of National Certification Examination for Registered Occupational
• Current Arizona state licensure
