The MDS Assessment Nurse:
Must be a registered or licensed vocational nurse in the State of Texas with a current license
Three years of MDS/Assessment experience
Must possess leadership ability and the willingness to cooperate with other department heads
Ability to organize, plan and direct nursing service
Must possess proficient computer skills.
Ability to maintain confidentiality in all resident and staff matters
Valid Driver’s License

Sits, stands, bends, stoops and walks intermittently during the workday
May push and pull carts and wheelchairs
Ability to lift moderate weight

Assist in scheduling of MDS assessments to ensure compliance with federal and state requirements
Review and certify completion of MDS assessments
Transmit MDS assessments to secure data repositories as required by federal and state regulations

Schedule resident MDS assessments to ensure timeliness.
Ensure timely completion of MDSs.
Participate in the preadmission process to determine potential resident’s level of care
Participate in resident level of care determination upon admission
Electronically transmit MDS assessments/tracking changes in resident
Collect data necessary to complete MDS forms and related documentation
Participate in interdisciplinary team meetings as needed.
Prepare Baseline Care Plan of Care within 14 days of admission and discuss with resident and/or responsible party.
Conduct interdisciplinary resident care conferences.
Perform resident care duties as may be required or requested by management
Observe infection control procedures relating to nursing.
Monitor and assist with compliance with policies and procedures to ensure compliance with state and federal regulations.
