We run the Del Mar Caregiver Resource Center, Multipurpose Senior Services Program and the Central Coast Area Health Education Center. Our programs support over 1,300 families annually to live and age at home at a fraction of the cost of an institution.

Nurse Care Manager– 32 hours/week Hybrid with $2,000 Signing Bonus

Duties
The nurse care manager is responsible for providing care management to participants to assist them to remain safely at home. The nurse care manager responsibilities include the following activities:
- Conduct in-depth home health assessments covering health, psychosocial, rehabilitation, and environmental concerns.
- Certifying a participant’s level of care: verifying they meet the requirements for the program.
- Assess the level of care requirements.
- Develop an individualized care plan for participant needs including emergency services, food, health care, medical attention, health education, emergency and permanent housing, mental-health, and substance-abuse counseling, as well as emotional and practical support.
- Coordinate participant needs for local, state, and federal eligibility, benefits, and resources through direct provision, education, and referrals.
- Monitor participant progress as outlined in their individualized care plan.
- Assist participants to manage medications.
- Participate in interdisciplinary team meetings.
- Utilize an electronic case recording system to provide relevant chart notes in a timely manner.
- Acts as a participant advocate to health care providers.
- Travel between Santa Cruz and Monterey Counties. Mileage will be reimbursed at $0.67/mile.
- Familiarity with local agencies and community resources a plus!
- Ease and familiarity with point of contact documentation (“Live Documentation”) directly into the participant's electronic health record while in the office and out in the field. Mandated Reporting: Knowledge of elder and dependent adult abuse warning signs and risk factors.
- Comfort with setting multiple participant visitations throughout the day. No emergency or on-call requirements.
Qualifications
- A valid current license (RN) to practice nursing in the State of California.
- A positive and healthy attitude toward participants, the agency and its staff and collaborating programs.
- Ability to travel within Santa Cruz and Monterey Counties.
- Capacity to work effectively with communities of color, the LGBTQ community, and people with HIV disease.
- Willingness to meet participants where they live (post-COVID)—this may include encountering unsanitary conditions and entering high-crime communities.
- Current CA driver’s license and a reliable, insured vehicle.
- Fluency in both English and Spanish preferred but not required.

- Starting salary is $39.00-$47.00.
